Grant Face Sheet Instructions

1. The deadline to submit the OUSD Grant Face Sheet is Tuesday, January 3, 2006 from 9 a.m. until 12 p.m. All OUSD Grant Face Sheets that have been signed by the each school site principal participating in the program must be submitted with a draft grant narrative, a completed budget, and completed OFCY forms to Mary Ann Burke, OUSD Grants Coordinator, in Room 312 located in the main administration building at 1025 Second Avenue. 2. For any application submitted for the comprehensive after school school-based programs funding, including by not limited to sites with a 21st Century Learning Center Program, an ASESP program, or a Healthy Start Program, the program development must be coordinated through Tanya Avila, OUSD's Extended Day Coordinator. 3. Data requests will be coordinated through the Grants Office. Rhoda Freelon (Rhoda.freelon@ousd.k12.ca.us; 510-879-8856) from the Research and Assessment Department will provide the data support for these requests.

4. From 1 until 4 p.m. on January 12, 2006, copies of the completed Grant Face Sheet can be picked up by an organization when a copy of the final grant application to OFCY is submitted to the Grants Office Coordinator. After that date, copies of completed Grants Face Sheets will be provided by appointment by contacting the Grants Office Coordinator at 879-8826. All OFCY grant applications for services provided at OUSD school sites will be prepared for review at an upcoming OUSD board meeting.

5. OUSD's indirect cost rate for the 2005-2006 school year will be 6.49%. This is NOT in excess of OFCY's 10%-15% indirect cost rate. OFCY's indirect cost rate must never exceed 10% or 15%.

6. All Oakland Charter Schools will use the OUSD Grant Face Sheet process as their Letter of Agreement with OUSD.

8. OFCY will accept copies of the submitted Grant Face Sheet at the time the OFCY proposal is submitted on January 9, 2006. The grant applicant will be responsible for faxing or mailing OFCY an approved copy.

OFCY DOES NOT REQUIRE THAT YOU SUBMIT PROOF OF INSURANCE OR FINGERPRINTS FOR THE SUBMISSION OF THE RFP. THAT IS AN OUSD REQUIREMENT.
